What is Care Coordination?Care Coordination is a personalized service that ensures all parts of your care are working together, especially when you're seeing more than one provider or managing multiple health concerns. At Resilient Mind Care, our Registered Nurse (RN) provides care coordination by: Helping you understand your health needs and treatment options Communicating with your medical, mental health, and wellness providers Making sure your services are not duplicated, overlooked, or misaligned Supporting you in making informed, confident decisions about your care Offering guidance on the next steps, referrals, or community resources Think of care coordination as your bridge between providers. What is your cancellation policy?We require 24-hour advance notice to cancel or reschedule an appointment. No-shows or late cancellations may incur a fee, as outlined in our intake agreement.

What are your rates?We offer both insurance-based and self-pay options. Private pay rates vary by service: Individual Therapy: $110 per session Couples & Family Therapy: $120 per session Faith-Based Counseling: $75 per session Coaching: $85 per session Group Therapy: $50 per session We also offer sliding scale rates for clients who qualify based on financial need.
